ROYAL COLLEGE OF GENERAL PRACTITIONERS

Our doctors are members of the Royal College of General Practitioners, with Dr Ware having obtained his Fellowship in 1997. We feel this represents the highest standards of general practice possible. We strive to continue to maintain this level of care for our patients.

COMPLAINTS PROCEDURE

If you are not happy with the service you receive, please ask to speak to the Practice Manager. If your complaint cannot be resolved informally they will record it in writing and ensure that a full investigation is carried out. Alternatively you may send a letter. Once a written complaint has been received, it will be acknowledged within 3 working days, an investigation will be carried out (which may include an interview with the complainant), the results will be discussed by the doctors and a formal response issued normally within a further 10 working days.

If you are unable to resolve your complaint through the practice complaints procedure, you may contact the complaints team at Lincolnshire Teaching Primary Care Trust, Cross O'Cliff, Bracebridge Heath, Lincoln LN4 2HN. Telephone: 01522 513355.

PATIENT CONFIDENTIALITY

We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible advice and care.

The information is only available to those involved in your care and you should never be asked for your personal medical information by anyone not involved in your care. You have a right to know what information we hold about you. If you would like to see your records, please call our Practice Manager.

CONFIDENTIALITY FOR UNDER 16's

Teenagers are entitled to the same confidentiality as adults; this includes children over 13 years of age providing the doctor or nurse feels they are capable of making decisions about their own health.

CARERS

We are interested in identifying carers, especially those people who, whatever their age, may be caring without help or support. We know that carers are often 'hidden' looking after a family member or helping a friend or neighbour with day to day tasks and may not see themselves as a carer.

Caring for someone is an important and valuable role in the community which is often a 24-hour job that can be very demanding and isolating for the carer. Carers should receive appropriate support by way of access to information on a range of topics such as entitlement to benefits and respite care and not least a listening ear when things get too much.

As a carer you are also entitled to have your needs assessed by Social Services. A Carer's Assessment is a chance to talk about your needs as a carer and the possible ways help could be given. It also looks at the needs of the person you care for. There is no charge for an assessment. Please ask reception for a Carers Identification Form or download now by clicking here.

PATIENT PARTICIPATION GROUP

Do you have a couple of hours to spare and would like to help us improve patient care? Come along to the above group. Leave your name and contact details at reception and we will let you know when and where the next meeting is to take place.
